A. Made to the health officer in such form as he may prescribe, setting forth such information as he may reasonably require to secure the purposes of this chapter.

B. Signed by the applicant and, unless for the abandonment or destruction of a well, accompanied by a filing fee of twenty-five dollars, no part of which shall be refundable. (Ord. 76-5 § 1, 1976)
